Data Architecture for Digital Health and Telemedicine
The Challenge
A leading international digital platform — which selects and brings together the best specialist doctors and private healthcare centres — had the ambitious objective of building a high-value data architecture that would enable the creation of advanced analytical services for clinical environments. The main challenge lay in interoperability and security: it was necessary to integrate heterogeneous data sources (HIS systems, laboratory results, IoT devices) operating under different standards (HL7, FHIR, OpenEHR).
Furthermore, the solution had to support a multi-clinic (multi-tenant) model, guaranteeing strict data separation, granular patient consent management, and rigorous regulatory compliance — all while laying the groundwork for future Artificial Intelligence and federated learning models.
The Solution
GALEO designed a healthcare data platform architecture (“IT for Health”) centred on standardisation and the creation of Data Products.
The solution is defined by a clinical data integration and governance strategy:
- Ingestion and Standardisation: Transformation mechanisms were implemented to convert raw data from HIS systems and laboratories into international interoperability standards such as FHIR, HL7, and OpenEHR, ensuring that information is readable and processable across different systems.
- “Golden” Zone of Data Products: Creation of an apified data access layer that serves as the single source of truth for patient applications, telemedicine portals, and reporting tools — guaranteeing the quality and consistency of information.
- Multi-Tenant Architecture and Security: Design of a logical and physical partitioning model to manage multiple clinics and hospitals securely. Data Governance and Data Catalogue modules were integrated to ensure full traceability, GDPR compliance, and consent management.
- AI Readiness: The infrastructure includes capabilities for deploying data science models and a Federated Learning API, enabling algorithms to be trained without compromising the privacy of sensitive data.

Business Impact
The project has provided TopDoctors with a technological infrastructure capable of scaling their digital health services.
- Real Interoperability: Ability to connect disparate clinical ecosystems (hospitals, laboratories, IoT devices) under a common language, facilitating care continuity.
- Service Innovation: The platform enables the deployment of advanced telemedicine applications and predictive analytics, underpinned by the availability of structured, high-quality data.
- Trust and Compliance: A robust governance framework ensures that the handling of sensitive data meets the strictest regulatory requirements — a critical asset in the healthcare sector.
- Scalability: The multi-tenant model allows TopDoctors to onboard new clinics and hospitals into their network in an agile and secure manner.
Architecture & Technology
The solution is built on advanced public cloud services with a strong focus on healthcare standards.
- Core Cloud: Google Cloud Platform (GCP) with Cloud Healthcare API.
- Health Standards: FHIR, HL7, OpenEHR, ISO 13606.
- Data & Analytics: Data Product Architecture, Federated Learning API, Data Catalog.
- Integration: Public API Gateway, connectors for HIS systems and IoT devices.
